The Formula
had + been + verb-ing
Take the past perfect (had + V3) and swap the participle for been + -ing. The result times how long something had been going on.

Common Mistakes
Three backstory slips
The long backstory fails when the -ing drops out, when the cause forgets its had, or when the present tense tries to enter a fully past story.
I had been work all day when you called.
I had been working all day when you called.
💡 The -ing belongs in this tense.
She was tired because she worked all night.
She was tired because she had been working all night.
💡 A cause that went on before the past moment takes had been + -ing.
He has been working here for years when I met him.
He had been working here for years when I met him.
💡 The whole story is in the past → had been, not has been.
